Membership Details
The membership fee will be billed in 12 equal monthly installments of $140 + sales tax.
Treatment sessions must be scheduled at intervals recommended by our medical professionals for optimal results.
Sessions cannot be transferred or redeemed for cash value.
The membership covers specifically the forehead, frown lines, and crow's feet areas.
Additional treatments or services outside the membership coverage will be discounted at 10% off.
Membership auto-renews annually; cancellation requests must be made 30 days before the renewal date.
This membership is non-transferable and applies only to the named member.
